How do I join?
You will need to complete a Dentist/Patient
Agreement, which will require your signature and bank
account details for the Direct Debit Mandate. Direct
Debits are collected from your account on the 8th
of every month. Any patient enrolling before the last
working day of each month will benefit from the cover
the 8th of the following month.
